NZDF Regular Force and Active Reserve personnel.

What is Complex Care?

Complex care includes situations where:

  • Non-clinical demands of case management exceed the resources of the primary care team to provide
  • There are multiple external and/or internal stakeholders

Includes physical and psychological illness. For example:

  • Serious injuries;
  • Recurrent historical / chronic injuries;
  • Terminal illness / Palliative care;
  • Acute medical events / diagnosis;
  • Concussion / Severe Traumatic Brain Injury (TBI);
  • Complex mental health;
  • Prolonged medical downgrade; and
  • Complex surgery

What do Complex Care Coordinators do?

  • Act as the single point of contact for the patient / whānau for assistance, support and
    guidance during the serving member’s health journey;
  • Point of contact with all key stakeholders;
  • Follows MDT (Multidisciplinary Team) principles / recommendations of care;
  • Ensures that personnel are effectively receiving clear communication and coordinated support tailored to their needs;
  • Provides advice, information, clear communication lines and wrap around support;
  • Provides Command advice, recommendations and updates;
  • Understands the support and services available,
  • Assists with navigation of the New Zealand health services; and
  • Ensures the patient receives all entitlements and financial supports.
